Rent our holter monitor

The Atlanta Doberman Pinscher Club makes our Holter monitor available for rent to anyone who lives within the state of Georgia, even if you are not a club member. The monitor is rented to club members, regardless of where they live, at no charge. Following is an overview of the equipment, required accessories and our rental process: 

EQUIPMENT INCLUDED IN LEASE AGREEMENT:

• Alba DR200 Holter monitor with vest, pouch & straps
• (1) set of leads
• (1) SM card
• (1) SM card reader
• Instructions and accompanying documents

PURCHASE OF ACCESSORIES KIT:

Applicants for rental of the Holter monitor must show proof of purchase of the required number of accessory kits from Alba Medical prior to initiating the rental process. Each kit includes:

• (1) AA battery
• (1) package of electrodes
• (1) patient diary

RENTAL PROCESS:

• Once rental agreement is executed, the Holter monitor equipment will be shipped via a major carrier (USPS, UPS, etc.) or hand-delivered.
• Renter pays shipping costs & insurance fees .
• Equipment (monitor, vest, instructions) must be returned within one week of agreed-upon return date; late fees apply.
• Renter is responsible for keeping the unit in good condition, for proper use and for cleaning the unit in accordance with instructions prior to returning.
• Renter is responsible for reimbursing replacement value if unit is damaged or lost.

PRICING: 

For Club Members:

• No charge for rental
• Shipping and insurance

For Non-Club Members:

• $70 per dog for one dog
• $60 per dog for two dogs
• $50 each for three or more dogs
